Inclusion Management is an NDIS registered service provider, delivering high quality and person centred supports to NDIS participants. All of our services are delivered by skilled, experienced and passionate staff who are committed to working alongside you to support you to achieve your goals.

SUPPORT SERVICES

Assistance with daily life.

Assistance with daily living supports are supports designed to help you with personal activities of daily life. Some of these activities could include:

  • Meal preparation

  • Personal care tasks such as bathing and dressing

  • Medication assistance

  • Assistance with attending personal appointments

  • Doing shopping

This support is funded from the NDIS Core supports funding category.

Your Inclusion Management support worker will work alongside you to ensure you are feeling supported with your daily living activities and needs. Your support shifts will be tailor made and designed to meet your individual needs and support your individual choices.

All Inclusion Management staff providing this support are fully qualified with all required clearances and with a genuine passion and commitment to support our participants.

Social & Community Participation

Assistance with social and community participation supports are supports designed to help you take part in social or recreational activities within your community. Some of these activities could include:

  • Supporting you to join and attend a social group

  • Going for an outing

  • Going to a community event

  • See a movie or a concert

  • Visit your local library or sporting club 

This support is funded from the NDIS Core supports funding category.

Your Inclusion Management support worker will work alongside you to support your goals of participating in social and community events. They can (if you choose) transport you to and from these and can also work alongside you to plan some outings for you to attend. Your support shifts will be tailor made and designed to meet your individual needs and support your individual choices.

All Inclusion Management staff providing this support are fully qualified with all required clearances and with a genuine passion and commitment to support our participants.

Group Activities

Inclusion Management have recently set out an area of our office designed to be a warm, inviting space for participants to visit to meet others, make friends and have some fun with one of our arts and crafts activities; playing one of our mixed selections of board games; joining us for a round of karaoke or even just to “chill out”.

SUPPORT COORDINATION

Support coordination is a capacity building support which helps you to understand and use your NDIS plan; connect you with services and supports and build your confidence and skills to use and coordinate your supports.  At Inclusion management we are registered to offer Level 2 Support Coordination, and Level 3 Specialist Support Coordination services.

Our experienced and dedicated Support Coordination team are committed to helping each individual to ensure you are getting maximum value from the supports in your plan.

Your support coordinator can help you to:

• Understand and use your NDIS plan to pursue your goals
• Connect you with NDIS providers, community, mainstream and other government services
• Build your confidence and skills to use and coordinate your supports.

Inclusion Management are registered to provide the following Support Coordination services:

Level 2 Support Coordination:

This support will assist you to build the skills you need to understand and use your plan. A support coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently, and be included in your community.

Level 3 Support Coordination:

 This is a higher level of support coordination. It is for people whose situations are more complex and who need specialist support. A specialist Support
Coordinator will assist you to manage challenges in your support environment and ensuring consistent delivery of service.

School Leavers Employment Supports (SLES)

SLES or School leaver Employment Support, is designed to support you to become prepared for work, otherwise known as job ready. This means you are ready to enter the world of work. Being prepared gives you the best chance of success in finding a job, learning a job, and keeping a job.

At Inclusion Management we will work with you to create an Individual SLES plan that maps out your employment goal and how to get there, what you might need support with and activities you may need to complete, so you are ready and equipped to enter the workforce.

Inclusion Management will work with you every step of the way; from career counselling and realistic job identification, creating and building a resume, interview skills and techniques, supported work experiences, workplace assessments, job searching and job applications, and on the job support.

When your SLES program is complete, Inclusion Management will work with you to link in with a Disability Employment Service who will continue to support you to maintain employment or continue to job search with you.

Inclusion Management staff have extensive disability employment experience within the DES sector and School to Work Transition Services. You will receive support from an organisation with over 40 years of combined experience. You can have confidence that Inclusion Management has the qualifications, working knowledge and experience to help guide you towards achieving your SLES goals.

Supported Independent Living

What is supported independent living?

Supported independent living (SIL) is a National Disability Insurance Scheme support. With SIL, NDIS participants or people with disabilities live alone or in a shared house. Inclusion Management can provide 24/7 care depending on the level of support required.

What support will I receive?

Inclusion Management will provide help and /or supervision of daily tasks to help you live independently as possible, while building your skills. SIL happens in your home-usually in a home you share with other NDIS participants.

We assist you with;

  • Cleaning and laundry,

  • Nutrition and cooking,

  • Personal care (such as showering and dressing),

  • Going to appointments,

  • Medication assistance,

  • Taking up hobbies,

  • Learning to catch public transport,

  • Community inclusion and social participation,

  • Skill development, and

  • Active support.

What are the houses like?

Our homes are carefully chosen.

Who will I live with?

Inclusion Management will help you find the right person to live with. This will be your choice regarding age, similar interests, and gender.

What qualifications do your staff have?

Our staff are qualified in the disability sector and have all the clearances required to work under the NDIS rules. We are a registered provider and get audited regarding our provided supports.

All qualifications are in line with the roster of care.

Can family be involved in the process?

Our team works in close collaboration with the individual, their families, and caregivers to identify the most suitable living arrangements that are beneficial for all parties involved. Working collaborative together we can provide a supportive and empowering living experience for our participants.
